SUMMARY:

• In compliance with Medicaid and COA standards, completes thorough mental health and developmental assessment of children, and develops and implements rehabilitative service plans, including both clinical and case management components.  Advises program management and direct care staff on developmental needs of children and assists in milieu planning and activities.  Advises on placement needs of children and works with discharge planning staff both inside and outside the agency to ensure appropriate placements and sound transitions.
  

QUALIFICATIONS

• Master’s degree in social work or other human service degree is required. At least one year of experience working with children and families is required.
• The employee must be LCSW or LCPC eligible, LCSW or LCPC license preferred.
• The employee should be able to drive a car and must possess a valid driver’s license.
